We get it, life happens! Your appointment and time is valued, as your provider, we respectfully ask that you mutually value ours as well. Please acknowledge that you agree to these terms any time you book an appointment with ZAQ Aesthetics. We will require a valid card be on file to book any appointments, this will retain your appointment. No charges will be made to this card unless in the event of a late cancellation or no-call/no-show. We ask that you please provide a 24 hour notice minimum to any cancellations or reschedules that you may need to make. Failure to comply will result in a first offense fee of $25, second offense fee of $50, and a third offense will result as a client dismissal from ZAQ Aesthetics. A no-call/no-show will result in 100% charge of the booked service, this amount cannot be applied to future services and must be paid in full before booking your next appointment.

Please arrive to your scheduled appointments on time. ZAQ Aesthetics will allow a 15 minute tardy grace period, however we may have to make adjustments to your service at that time to still finish on-time as there can be another appointment following yours.

First time appointments, please make sure you fill out your intake forms prior to appointment time. Come see us with an open mind and don't worry about not knowing much, we will come up with a plan special to you! If you are using any retinols or topical creams, please stop using a minimum of 3 days prior to your service.

100% YES. It allows us to gauge your sun exposure, physical and mental stress which all effect your skin. Your medications are also super important for us to know so we know what potential side effects could happen and what products to avoid. Pregnancy is also extremely important so we can make sure we use all pregnancy friendly products and ingredients. Please know that any personal information you provide is confidential, even if you haven't told anybody, we still need to know for your safety. Allergies are also included in this, even if you think it doesn’t pertain to your skin please still write it down. Skincare ingredients include a lot more than you may think, as the professional it is our job to know what can be a potential contraindication. Recent injections (botox/filler) we want to know so we do not disturb anything while it is settling in place.
